Innovative solution to revolutionise preventative heart healthcare
We are proud to announce that HCF, Australia’s largest not-for-profit health insurer, will deliver Cardihab’s Primary Prevention program – designed to transform preventative cardiac care for HCF members that have a moderate-high risk of CVD.
This program will enable HCF members to access Cardihab’s digital home-based cardiac prevention program empowering them to proactively take control of their heart health with a supervised health coach, personalised, and evidence-based care plan.
This program marks a significant step in addressing the rising burden of cardiovascular disease as 1 in 6 Australians suffering from heart related conditions. This initiative aims to improve access to preventative cardiac care, reducing barriers to primary healthcare services while empowering HCF members to understand how to reduce their risk of CVD and how to live better healthier lives.
