The secure Clinician Portal is a one-stop-shop for tracking and monitoring how patients are progressing through your cardiac rehabilitation program.

Once the patient is enrolled in the system all authorised care team members can access that patient’s program in the portal via intuitive patient dashboards.

The Clinician Portal provides an electronic record of program completion necessary for institutional reporting requirements and patient discharge summaries.

Cardihab enables people to receive patient-centred care from home to help overcome common barriers to participation, and provide patients with the care they need in ways that work for them.

As there are no capacity constraints more patients can participate, and they can commence as soon as referred. By servicing more patients, sooner, hospitals should see fewer cardiac readmissions - the ultimate measure of success.

Cardihab’s digital platform has been scientifically validated to match or exceed outcomes of traditional Cardiac Rehabilitation.  Offering digital cardiac rehabilitation has been shown to improve participation and completion rates over traditional models of care.

The research providing the evidence base for Cardihab's product and was published in the prestigious journal Heart. The paper was also acknowledged by Heart and Nature Reviews Cardiology as a world-first randomised controlled trial of smartphone-based cardiac rehabilitation.
