About the Primary Prevention program
Covered by HCF and delivered by Cardihab. Eligible HCF members* who have recently had a heart health check can access the Primary Prevention Program. It’s designed to help you understand the things you can do to reduce your cardiovascular disease risk, and prevent complications or events such as a heart attack or stroke from happening.
Your program is tailored to you, giving you the flexibility to complete it at a place and time that suits you and your lifestyle.

How to get started on the program
Eligibility and what you’ll need
Before you register, make sure you’ve held HCF hospital cover that includes heart conditions and vascular system cover for at least 12 months, and have your HCF membership number and Provider Code HCFPPP ready.
