About our Primary Prevention Program

Heart disease is a common health concern, affecting up to 1 in 6 Australians1. Fortunately, small but meaningful changes in lifestyle, diet, and exercise can significantly improve heart health.

For people that have had a heart health check and discovered they are at higher risk of heart disease, the right program can make a meaningful difference to heart health. This includes information on the choices you can make to improve your heart health, as well as advice from a qualified healthcare professional regarding your risk factors.2

How it Works

  • A personalised care program delivered virtually.
  • A qualified care team—providing expert guidance and support.
  • The Cardihab SmartCR mobile app—delivering daily tasks, educational content, medication reminders and tools to track participant progress.

Through the app, participants can:

  • Log exercise, record health metrics (e.g., weight, blood pressure)
  • Set medication reminders and track adherence
  • Record symptoms, helping the care team adjust the plan as needed

The care team monitors participant progress via a secure portal, ensuring timely support and intervention.

Together, these tools empower participants to take control of their heart health and improve their long-term well-being.

Using the knowledge and tools learned during the program, participants can continue independently with the app to keep on track.
