Australia’s leading not-for-profit health fund HCF to offer Cardihab’s digital cardiac care solution to members
Innovative solution to make preventative heart healthcare more accessible
We’re proud to announce that HCF, Australia’s largest not-for-profit health fund, will deliver Cardihab’s Primary Prevention program – to support eligible HCF members at moderate to high risk of cardiovascular disease (CVD).
Through the program, eligible HCF members will be able to access Cardihab’s digital home-based cardiac prevention program, designed to help them take proactive steps to manage their heart health. Participants will receive personalised, evidence-based care plans and one-on-one support from a dedicated HCF Health coach.
The initiative responds to the rising burden of cardiovascular disease in Australia, with 1 in 6 Australians currently living with heart related conditions. By improving access to preventative cardiac care, this program aims to help HCF members make sustainable, healthy lifestyle changes while reducing barriers to primary healthcare services – especially for those in rural and remote areas.
Our purpose-built digital health solution for cardiac disease prevention is available to HCF members from today, and can be accessed conveniently via mobile, table, desktop or wearable devices – wherever and whenever members need it.

- One-on-one support from a dedicated HCF Health coach
- Tools to help set and track healthy living goals, including physical activity, nutrition, and stress management
- Medication management to encourage treatment adherence
- Equitable accessfor members in regional, rural and metropolitan areas
- Convenient and flexible care via mobile, tablet, desktop, or wearables
