Westfund, Honeysuckle Health and Cardihab Expand Access to Cardiovascular Care
Cardihab is pleased to announce a partnership with Westfund Health Insurance and Honeysuckle Health to deliver digitally enabled cardiovascular programs for Westfund members, supporting both primary prevention and cardiac rehabilitation.
Cardiovascular disease remains one of the leading causes of hospitalisation in Australia. Evidence shows that early intervention through prevention programs, combined with structured cardiac rehabilitation following a cardiac event, significantly improves outcomes and reduces long-term health system burden.
This collaboration brings together Westfund’s commitment to member health, Honeysuckle Health’s clinical coordination, and Cardihab’s evidence-based digital therapeutics to support members across the full cardiovascular care continuum.
A connected model of cardiovascular care
Eligible Westfund members can access two complementary programs:
Primary Prevention Program
Designed for members at risk of cardiovascular disease, the Primary Prevention program focuses on early intervention and risk reduction through:
· Personalised lifestyle and activity guidance
· Education to support behaviour change and heart-healthy habits
· Ongoing digital engagement to build sustainable routines
Cardiac Rehabilitation Program
For members recovering from a cardiac event or procedure, the Cardiac Rehabilitation program provides:
· Clinically supervised, guideline-aligned rehabilitation
· Individualised exercise, education and lifestyle support
· Digital delivery via the Cardihab app, enabling access regardless of location
Together, these programs support members before and after a cardiac event - shifting care upstream while ensuring structured recovery when it is needed most.
Improving access, engagement and outcomes
Digital delivery removes many of the traditional barriers to participation, including geography, time constraints and limited program availability. Clinicians can monitor progress, tailor programs, and support participants throughout their journey, strengthening continuity of care and long-term engagement.
